Adding web resource broke new look sales process form

  I added a Multi-select field using this updated managed resource (http://taoofcrm.com/2013/01/15/multi-select-option-sets-part-2/#comment-577) to my new look Opportunity Form (rollup 12). On the required web resource, I did not select "show this web resource in read optimized form", which subsequently broke the new look form. I have not been able to get the form to display correctly again, even after checking the aforementioned setting or removing the web resource. Any tips on how to revert the form to the correct format?
