Can not promote a phone call activity to a campaign response 2011

Hello Experts,
i 've been having issues to promote a phone call activity to a campaign response by clicking on the ribbon button "promote to campaign response". First all want to say that i have the "Admin" role and i filled all the required field thar are needed to activite the ribbon button "promote to campaign response".
After clicking the button, a campaign response will be generated by the system but the values of some field will be empty. Concerned fields are:
- parent campaign
- Customer
- originating Activity
I have no scripts , nor plugins or workflows running on the Webform campaign response, Phone call activity, campaign activity or campaign. So i am wondering where does the bug come from? Those three are not custom fields and normally the value should be automatically populated.
Can someone please tell where the bug is? what is the cause of this strange behavior?

Hi
no need to a mapping, th system should automatically populate the value from the phone call to the campaign response. Besides between phone call and campaign respson is a 1:N relationship from type : System that's mean i can not manually map the fields of the two entity.
I've created an extra Lookup field between both entities and noticed that the Micrsft programm wass no more able to work properly . I've remove the lookup field and everything is working fine, but don't why

Hi,
Your phone call activities, were they created from a campaign activity? If they were, those 3 fields should be populated, otherwise CRM doesn't know the originating parent campaign, customer etc and relies on the user entering the info.

Hi Bob,
the phone call activities were created by a telefon activity. The problem occurs when i created another relation between both entities from typ 1:m (phone call : campaign response) fromt typ referential because i first at all wanted to mapped the all the custom fields of both entities and in the phone call form i want to have a subgrid for campaign response.
it seems like by creating theis new relationship, microsoft script doesn't longer run properly don't know why?
Secondly is there a way to create a subgrif of campaign response in the phone call form?
In the phone call form customization these are the entities that i can create a subgrid on it, the entity camaping response doesn't appear in the list , although there is a 1:n relationship between botth entities
