    Hi Guys,

    Is it possible that on the "Confirm Deactivation" window of a record in Dynamics 365 to have a Text field that captures the "Deactivation Description" when the status reason is a certain value?
    Image below:

    

    When status is a certain value, we want a text field to be displayed to capture deactivation description.

    Any ideas on how to go about accomplishing this task?
