Holding MCSE(2003) upgrade to MCITP

I'm currently MCSE (2003) certification. I would like to upgrade my MCSE to MCITP. What are the exams to write to upgrade from MCSE to MCITP. Please reply me with required exam code for MCITP.
For upgradation, If i finish only 70-649 exam, which certificate i will get from microsoft.

1)There are few upgrade paths, which depends on type of MCITP more detailed here http://www.microsoft.com/learning/en/us/certification/mcse.aspx#tab4

2) You will receive TS:Upgrading your MCSE from Windows 2003 to Windows 2008 ;)
