.Net 4.6 asp.net application migration to .Net core platform

  We have an existing asp.net application currently running on .Net 4.6 framework and now planning to rebuild the same application on .Net core platform. Is there any Microsoft tool or solution available which can help us in transformation?
  
  Help should include what all libraries will be replaced, what code changes are required, database related changes etc etc.
