Deleting accounts from CRM resulting in duplicating the account.

I have an account in CRM that I want to delete but i'm unable. each time I try to delete one account it creates the same account over 100+ times.
Any advise on how to work around this ?

I think the issue is caused by ADFS since I installed it along with CRM on the same server. I have removed it from there and issue was resolved.

Check for a pre or post plugin on account entity? Is this behaviour only for a particular account?

It happened to 2 accounts already and I have one of them duplicated over 5000 times. I think i'll just format and re-install this CRM machine coz it's full of troubles already. and I wasn't the one who set it up.
i'll postpone this question while I do the installation from scratch and if I have any question i'll re-port. thanks a lot for helping Imran.

Hi Imran,
I have reinstalled CRM and I got stuck into the problem of connecting CRM client on my PC to the CRM server on premise. I had this problem with the server before the reinstallation. except that it only worked on my PC not anyone else.
I read in many topics that its related to Bindings in IIS. I have checked the binding and there's only one value there and its identical to what's in the Deployment and the same thing applies for the port too.
I have restarted the server 10 times and it's fully updated. do you know if there's anything else I can do to solve this issue?
