You can create a Campaign Response in CRM to quickly see the fields required or do an Advanced Find on Campaign Responses and select edit columns to see the fields available. Select the fields you need from Advanced Find and run a query that has
a filter option that will return no campaign response results. The reason for this is that you want to generate the header row for your campaign import. Export the result to Excel and save the header row for use with your csv file import.
Now you can copy the header row to your campaign responses and you should be able to easily import your campaign responses.
