CRM 2011 Outlook integration issue - Appointments in Outlook not syncing with CRM Activities

Hi,
We have an issue where new users created since Aug 2014 are unable to have their Outlook appointments appear under CRM activities like they should after they "Set regarding" that appointment.
This is a feature that we have been using for years with no problem so it must be related to something that has changed since Aug 2014 - but just not sure where.
We used CRM 2011 On premise, and Outlook 2013 on Win 8.1. (Win 8.1 is the only thing I can think of that has been new since that time).
We are on Rollup 6 and 14. (Both old and new users are exactly the same).
Have been researching for Months with no result.
Anyone have any ideas?

Before going any further,there’s couple things I want to confirm with your first:
- Did the issue start in Aug. 2014 which also was the time “(Win 8.1 is the only thing I can think of that has been new since that time).” – I assume it was OS upgrade? And new users also were created around this time?
- “We are on Rollup 6 and 14. (Both old and new users are exactly the same).” Do you mean that you upgraded the OS to win 8.1 for 6 old user and 14 new user?
- Is this an issue occurring for new users?
- Is this issue occurring for certain appointments or all of the appointments?
- Are the Outlook accounts exchange accounts or other type of accounts?
If it a new users issue only, then it might be related with user permission, were the new users’ record added as a user to the Required or Optional field of the appointment might be the first thing we need to check.
If it’s an issue for certain appointments, do those appointments contain inline attachment? If so it won’t be supported in CRM, please read this article for detailed information.
Meanwhile, please make sure your OS/CRM/Office contain the latest update.

Thanks. Some responses as follows:

And new users also were created around this time? >>J: Yes, it was just an OS upgrade. Existing users were upgraded and new users went straight to Win 8.1. It is these new users with the problem.

(Both old and new users are exactly the same).” Do you mean that you upgraded the OS to win 8.1 for 6 old user and 14 new user? >>J: No, I meant all old and new users have the same version of CRM Outlook integration software loaded, and the same CRM Rollups (v.6 and v.14) specifically. We did try loading V18. on the new users, but this didn't fix the problem.
3. Is this an issue occurring for new users? >>J: Yes, all new users.
4. Is this issue occurring for certain appointments or all of the appointments? >>J: All appointments
5. Are the Outlook accounts exchange accounts or other type of accounts? >>J: Exchange accounts
