if OneCare is installed, the real time protection is on and running, so no other program needs to know what protection is being provided and by what program. Any file that is written to disk is being scanned by OneCare and any file that is being executed, that is launched or loaded into memory, is being scanned. If you have a download manager, for example, that calls for you to specify the virus scanner to scan downloaded files, OneCare cannot be selected here as that functionality is not available.
