Hi All,
I am trying to upgrade CRM 4.0 to 2011. But i get 2 errors
Violation of PRIMARY KEY constraint 'PK__#ActualC__59D4927546SFA08'. Cannot insert duplicate key in object 'dbo.#ActualColumns'.
Violation of PRIMARY KEY constraint 'PK__#4542R4C__73365VR4TT2BDR'. Cannot insert duplicate key in object 'dbo.@ActualUserTables'.

Maybe re-run it with sql profiler on to see what query it blew up on. That might help us figure out the problem.

only other thing I can think of is making sure you are on the newest UR in 4.0 going to 2011 and make sure you use UR 14 if you are going from 2011 to 2013

This is a weird one. Have you tried it at least twice and verified it keeps happening. Also verify that this org hasn't been already imported into this CRM 2011 instance.
Hmmm.. Lastly I would like to verify that there weren't any manual changes made to the database you are trying to upgrade beyond what is supported by the SDK and normal customization mechanisms for CRM.

I have tried 4 times and get same errors. I have not changed any think in database level. Just tried in a fresh CRM 2011.

I'm getting the same error when I run the 2013 setup while upgrading from crm 2011. So I'm going from 2011 to 2013 and I'm seing the same error except the second error is saying the violation occurred on "PK_#4fcd00_..."
Any Ideas?

It really looks like there have been made unsupported direct changes to the DB - maybe indexes or views
You should check with your DBA and delete unsupported modifications
