Hello Clyder,
The first section of the Diagnostic Report tells me that your Vista is not Activated, but Licensing Data section suggests that Vista's Licensing Store may be corrupt or is unreadable for some reason.
Would you try the following to recreate the Store:
1) Open an Internet Browser
2) Type %windir%\system32 into the browser address bar.
3) Find the file CMD.exe
4) Right-Click on CMD.exe and select 'Run as Administrator'
5) Type: net stop slsvc (it may ask you if you are sure, select yes)
6) Type: cd %windir%\ServiceProfiles\NetworkService\AppData\Roaming\Microsoft\SoftwareLicensing
7) Type: rename tokens.dat tokens.bar
8) Type: cd %windir%\system32
9) Type net start slsvc
10) Type: cscript slmgr.vbs -rilc (It may take a long time for this to complete, please be patient)
11) Reboot Twice
12) You will likely be asked to re-enter your Product Key and/or reactivate.

Additional notes: no access to start button or anything other than what is accessable from IE - but with limitations.
User had OS installed by a computer company - IDE 1 is where the OS is installed rather that IDE 0. Both drives are sata one at 500 gig other at 1 terabyte
previously had 0xc0000034 error used startup repair to correct the BCD store to get to this new issue.
Also tried a fresh install which I installed on IDE 0 - decided to ask first before instaling a fresh instal on IDE 1 (Not my choice)- Edited by Clyder B Tuesday, October 6, 2009 7:40 PM addtl info

Thanks - everything worked as advertised.
One last question . . . I now have vista installed on both hard disk drives. What is the correct way to remove the unwanted installation? And is there a way to confirm I am un-installing the correct one?
Would I be able to just re-format the drive the second intallation is on while in the correct installation? Could it be that easy?

Hi Darin,
I had the same issue as Clyder and used your technique above to re-install the license files. I was finally able to input my Product Code and log-in to windows. However, it now says that "This Copy of Windows is not genuine". I'm not sure where to go from here. This computer came with Vista about a year ago when I bought it and it hasn't had any problems until now.
