Just to follow this up, this is resolved by applying office 2003 update KB907417
Update for Office 2003 (KB907417)
Brief Description
Microsoft has released an update for Microsoft Office Excel 2003 and for Microsoft Office Word 2003. This update corrects a potential problem in which an add-in, a smart tag, or a smart document may not load if it uses the Microsoft .NET Framework 2.0.

Simon, maybe it's time to call your Microsoft Support channels. Start by running the Diagnostic Wizard so that you'll have specific info to give the tech.

Hi Simon,
Do you see any contacts in CRM address book?
It should be any error related to this issue in Applications logs in Event Viewer. paste it here and we will try to help.

Hi Simon,
Has this been resolved? If not, when you did the upgrade, did you first completely uninstall the CRM 3 Outlook Client to include removing folders, registry values if needed, etc.
If you used the Upgrade option instead of doing a new install, I suggest you uninstall and completely clean the machine of the CRM Outlook Client, all traces. You can find instructions for the manual removal in the Microsoft Knowledge Base.

Take a look at this article, it should provide the information you need to manually remove the client. Keep in mind that you may now be removing both 3.0 and 4.0 'pieces' so look for everything as best you can.

OK so here's some more info.
I've uninstalled CRM 4 Outlook client completely and removed all traces of CRM including the old CRM 3 address book provider. Then installed CRM 4 with Rollup 7.
The users settings for the Address Book are Contacts: Match all contacts in CRM and Other Items:Match all items in CRM. But the CRM Contacts are empty in the address book!
When we click "send to" in office we get 5 crashes from office, below are the details from the application log.
Here's hoping you guys can help.

I've run the Outlook Client diagnostics and got one warning which I'm unable to resolve.
"You recently used a different Outlook profile that the one Microsoft Dynamics CRM add-in has been configured for. The Microsoft Dynamics CRM add-in will be enabled only for the profile it was configured for."
Anyone got any idea's?
This is my last attempt before I go through the process of logging a call with Microsoft.

Hi Simon,
I think the CRM 4 Outlook add-in is added under the user account / outlook profile of the person who does the installation. Grant the user the appropriate to install the CRM 4 client / Outlook Add-in.
hope that helps

Hi Simon,
I don't think the message you received is an error. I think it is intended as a warning / informational message to let you know that whatever profile recently used to open Outlook was not the profile used to configure the Outlook client and the Outlook client will not be available for that recently used profile.

Hi Donna,Thanks for your input, the warning still worries me.The client is installed as an Local Admin, then logged in as required user, configured outlook, run diag and got the warning which is confusing when we were using the same account that outlook was configured with. There are multiple mail profiles on the machines and they do have roaming profiles, I'm trying to get the client to log a call with MS.On a clean machine that has not been upgraded there is no issues.

Hi Simon
Were you able to resolve this issue?
I am having the same issue on Office 2003 and CRM 4.0. (Not even an upgraded Outlook installation)
I'm going to log a support call with MS unless you have found a solution?Thursday, March 18, 2010 9:07 AM -
Hello,
I'm also getting the exact same problem on Office 2003 and CRM 4.0. Also a new installation, not an upgrade. In our case, a new profile has been created for the user, CRM client uninstalled, SQL uninstalled and CRM client reinstalled. Still no joy.
Has anyone logged a call for this yet, or have any new updates?

Hi all,
I have this same issue with a customer of ours. The machine has had a complete rebuild and still has the same issue. I have found a small workaround, but it removes a portion of functionality.
The issue is related to the CRM ABP or CRM Address Book Provider for Outlook, as is already mentioned above. The way I have found to get around this particular crashing issue, is to disable Address Book synchronisation which 'updates the Outlook Address Book with data from Microsoft Dynamics CRM so that names and e-mail addresses can be automatically resolved.'
The way to do this is to Open the Diagnostics tool (Start Menu, Programs, Microsoft Dynamics CRM 4.0, Diagnostics), the select 'Support Mode'. On the 'Synchronization Troubleshooting' tab, untick the 'Address Book Synchronization' check box, then choose Save and close the tool.
After re-opening Excel or Word and then trying again - you should now be able to successfully open the 'send mail' window with the attachment pre-attached.
I'm still looking into how to get this to work properly as this is a very ugly workaround, but this may help someone out there.
Regards
DanTuesday, April 6, 2010 2:07 PM -
Further investigations to add for diagnostic....
- Modifying the options in Outlook > CRM Menu > Options > Address Book tab ..... makes no difference to the issue. (Tried all possible combinations including restarting MS Outlook, CRM Client hoster and Excel each time.)
- The user experiencing the issue owns ZERO active contacts in CRM. Adding one makes no difference.
- The user has some distribution groups setup in his Outlook contacts - (not sure if this is related)
Next step is enabling Client Side Tracing. Will keep you all posted.
DanTuesday, April 6, 2010 3:02 PM -
Ok, enabling client side tracing has produced this error in the log
[2010-04-06 16:22:54.280] Process:EXCEL |Thread:0948 |Category: Unmanaged.Platform |User: PlatformUser |Level: Error | Error in CreateDOM: Create Managed Instance of XmlUtility, UNKNOWN (0x80131700)
After googling this last error code, I came across an article which points me at a missing or corrupted installation of 'Microsoft .NET Framework 1.1'. Tried installing that and rebooted - this hasn't helped either.
When I reconfigure this CRM client to point at another CRM deployment using a user account which I know doesn't have the issue on another workstation, it has the same issue so I know that the issue has to be local to the workstation.

Hi Daniel,
You don't happen to be using Office 2010 Beta release are you? I've seen this error in the Beta Software where the Outlook Connector was also installed. The Outlook Connector is used to sync other e-mail accounts like hotmail.
Best Regards | Twitter: edwardsdnaWednesday, April 7, 2010 12:50 PM -
Hi Donna,
Thanks for your reply!
I personally am using CRM with Office 2010 beta (and loving it!) but alas, the user who has the issue is using a fully patched Office 2007 with no Business Contact Manager, Outlook Connector or any other addin installed. Thanks for the idea though.
I was waiting to see if MS managed to release Rollup 10 today as was planned, before logging it with them. At the time of writing this though ........ hold on- It was released 39 minutes ago! How's that for spot on delivery?! Well done MS!!
Although the download has been released, the link to the knowledge base article fails - so I still don't know what is fixed in this rollup.
I'll check again tomorrow
