Issue with note subject online CRM 2013

Hello to Everyone,
Facing an issue with the notes entity record creation with online CRM 2013.
But I am unable to reproduce the issue as it happening with one of our client.
When a note record is being created from a tablet device using the mobile version of CRM the note subject is getting created automatically with a UTC format date time instead of normal date time. And it not happening foe few times. Like below format.
I assume the default note creation functionality has been removed from new version of CRM as it is a online CRM.
Is there any possibility the subject is still creating automatically? If yes is there any setting I need to change the format?
Any suggestion how can fix the issue.
