Hello salamain,
You will notice that under the "File Scan Data-->" line there are a large number of files that are listed as Mismatched. All those files listed are Critical Vista System files. When there is one or two Mismatched files, it means that those files listed have been tampered or corrupted in some way. But in your case the Diagnostic Report is reporting that ALL your Critical Vista System files are Tampered or Corrupted. This is highly unlikly. Whats more likly is that your System Catalog has been tampered or corrupted. The System Catalog stores all the Signatures Hash (think fingerprint) for all the Critical Vista System (and other) files. One of the things that Vista uses the System Catalog for is to identify if a file has been tampered or corupted because if the file had been modified in any way, it's signature hash would not match what is listed in the System Catalog.
Aside from having Vista show as non-genuine, this issue may cause Vista (and possibly other programs) to become unstable and/or not behave incorrectly. I highly recommend using the System Restore process that came with your computer. (how to conduct the system restore may be diffrent for each computer manufacturer, so please see the documentation, that came with your computer, for assistence in the restore process).
Unfortunetly, this is a destructive process, meaning all settings, files and programs (that were installed after the computer was built) will be lost. Vista will be restored to how it was when it came from the factory. I highly recommend backing up your importent files to removable storage (Disk, Removable Hard Drive...so on).
I am very sorry, I know my suggestion will not make you happy, but it is the only sure way I know of to fix your issue. If it's any consolation, this is a very rare issue (i've only seen it three times) so it should not happen to you again.

I am sorry, but I do not know. It could be from a Virus or Random file Corruption or you or another user may have deleted/modified the file by accident (to name just a few possible causes).

Good evening Darin,
I must break the border of just one user with a oleacc.dll message. I'm getting LOTS of those popups mentioning "this application can't be started because OLEACC.dll can't be found.
Well, OLEACC.dll is in my windows/system32 folder of Vista Ultimate x64 system.
I can find it, so why can't windows?
There's no message anywhere about wrong files or whatever, just this one message popping up for about any application i start.
What can be wrong? We had the PC on last night, this morning it was logged off (probably rebooted cuz of maintenance) and when my wife logged in the oleacc.dll message storm started.
Any help is highly appreciated.
