Hi Peter,
we had the same Problem on a customers environment and invested a huge amount of time to fix this problem.
The line that throws the error, trys to save the "Recently Visited Items" into a local file. Sounds reasonable, cause the line is save("RVItems");In our case the problem was combination of folderredirection and TerminalServices. In a short: this caused a missing folder in the user-profile, which caused the IE to say "unable to find file".
The folder is the following (on a Win 7 Box):
C:\Users\<USER>\AppData\Roaming\Microsoft\Internet Explorer\UserDataon XP or 2000 you have to look in "c:\documents and settings\...". I'm sure you will find it ;)
We just created the folder "UserData" via Windows-Explorer and then everything went fine...
Hope this will help you and some other readers ;)
Kind Regards

Hi Peter,
May I know if there are any scripts written in the form? I guess there is something in the script which is trying to access the file which is not accessible. Please let me know the status of it.
