Please explain the meaning of the "No media operations are allowed because the call is not connected" error?

  I am getting "No media operations are allowed because the call is not connected error" on the production logs and was wondering if anyone has any ideas when it happens and how to troubleshoot it. We are not able to replicate it in development.
