Every OFX/QFX transaction contains a unique identifier. That value is in the <FITiD> entry. Each one must be unique to that specific transaction. If the bank/brokerage that is generating the <FITID> value does not make it unique, then Money will
think that the match that it finds in the register is actually the same transaction as the new one and will not import it, thinking that it already exists and must not be duplicated.
This also explains why transactions that have already been imported will not be entered a second time if you try to re-import the same file.
If new transactions are not being imported, then it is the fault of the bank/brokerage for not creating unique values and re-using ones that they have already used before.
