Problem logging in with Authenticator App enabled

Chrome version 68.0.3440.106 (x64).
My LiveID account has two factor authentication enabled. When I try to login, it does send notification to my Android Authenticator App to approve. I approved it but somehow the server never get the response.
I tried to enter the OTP number, and then was told I cannot because I failed to type too many times.
Seems the login system needs a bugfix and probably redesign. (Why would sending notification to App without reply count as 1 failed attempt? And if it's not the case it'd be even more scary, as I'm told I failed too many times even before I tried)

I think it is caused by stale cached microsoft account credentials which may have eventually expired.

No, I've never used that Chrome browser to login before so there would not be cached login info.
Now think about this issue again, both time I login success, I have already had the Authenticator App opened in foreground when trying to login. So maybe it's not a browser problem, but more a problem related to how the Authenticator App react to the notification popup it spawned, and how the website decide whether the 2FA attempt is failed.

When this happens if you watch closely I think you'll see the logon attempt about 13 times in very fast succession which is where the too many repeated authentication attempts comes from.
