Create a custom web app. Use LDAP to let the users to change their password.
Here is sample app

Hello,
Unfortunately there is no such out-of-box feature. One possible way out is to develop this functionality.

Hello,
I have no experience in it because I'm developer for CRM and not admin for Exchange ;)
You can try to take something from Exhange but the easiest way (in the case you really need it) is to create custom application - for example based on the Silverlight.
