Has anyone upgraded from 4.0 to 2011 on the cloud? If so, is Outlook upgraded automatically like the Web client? Or is there a manual install required?

CRM Online will take care of the server side upgrade. For the Outlook Client upgrade - handled by the end user.
Here is the link to the CRM Outlook Client installation. I would recommend an uninstall of CRM Outlook 4.0 and a reinstall of CRM 2011:
