I just near deafened myself when i minimized a game to lower the volume and whilst i was moving the slider, a popup came up for a disk drive, and that somehow resulted in the slider flying straight up to 100. Mind you, i was moving the slider from 2 to 1,
because my headphones are very sensitive... so you can imagine how loud that was.
I have Realtek HD audio, ie the baseline audio chipset used in most Windows PCs and Windows 8. When i plug in a device, it asks me what device it is, then i don't see what happens as a result.
Please implement a volume limit to each device. When i swap from speakers to headphones, i expect the volume levels to change, because i use my speakers at about 70% volume. So if i swap to headphones from my speakers and forget to change the levels, i'm
surprised with a near deafening level of sound because it stays at 70%. Ideally all i want is that the system to remember what volume levels i was using on a per-device level, but a fail-safe is necessary to protect against unforseesable circumstances as well.
