With an on premise CRM install, is it possible to use the same URL for internal and external access (i.e. Internet) to CRM? Just wanting to see if it is practical and possible.

Yes it's possible, but it's much easier to always use the external access url as both the external and internal url(i.e. https://<orgname>.domain.com or https://<orgname>.subdomain.domain.com). If you need to change the external url, it can be a pretty big pain.
I should qualify this by saying that when you're on premise, you'll get your organization name appended on the end of your url (i.e. https://<orgname>.domain.com/<orgname>/...) while over IFD you'll just see https://<orgname>.domain.com/... This can cause problems if you have multiple organizations and are using the web client, since sometimes CRM will forward https://<orgnameA>.domain.com/ to https://domain.com/<orgnameX> which would mean you would need to use https://<orgname>.domain.com for IFD and use https://<orgname>.domain.com/<orgname>/ internally. This issue only applies to the web client, and not Outlook client setup.
