It looks like you got an access denied error (0x80004005) when starting AV. Can you detail a bit more on what you are trying to do?
Perhaps the meeting policy assigned to the "presenter" of this meeting lacks the rights to launch IP Audio and IP video? Can you check this?
Look here for more info:

See amsi658 above. The rtcproxyservice user must be english.
Please reply back if you still experience the same problem after changing the default language

I am currently experiencing the same behavior when attempting an external A/V conference. The link above mentions the default language issue that MS has solved in the April updates. ( and have applied all updates available to R2 (including KB967831).
Has anyone found a solution for this issue?

In my case I made a mistake in configuring A/V Authentication Service setting on OCS server.
My A/V Edge Server uses default ports.
In OCS Server Global Properties -> Edge Servers -> A/V Edge Servers when you add Edge Server "A/V Authentication Port" should be the same as "A/V Edge Server TLS Signaling Port" (but NOT "A/V Edge Server Internal Interface Port") on your Edge Server. To see that you need to run Deployment Wizard on your Edge Server and run "Configure Edge Server Wizard".Thursday, July 2, 2009 1:36 PM
