I fear that there is not much available so far. You can check the page of the exams to see the areas that are tested and read on MSDN / other pages on the internet. You can even look for books that cover some topics.
(And hopefully there will be a self paced training kit one day. But I never heared that it is planed :( The last MCPD book I know was the self-paced training kit for 70-549 (framework 2.0))
But you also found the old Thread where Geoff and Jorge tried to answer this question ...
(And maybe I think that the whole topic is to complex so that you have to know a lot of different topics e.g. different testing methods, n-tier applications, web farms, .... But I would like a book about all this, too!)
