The solution is to manually create the two directories, cHTML and xHTML, in the %programfiles%\Microsoft ISA Server\CookieAuthTemplate\Exchange folder. Then, copy the contents of the %programfiles%\Microsoft ISA Server\CookieAuthTemplate\Exchange\HTML folder to the cHTML and xHTML folders.

    Hi lawma :

Based on my research, you can enable forms-based authentication for OMA. For more information about OMA, you can refer to the following documents:

Exchange ActiveSync and Outlook Mobile Access errors occur when SSL or forms-based authentication is required for Exchange Server 2003

    http://support.microsoft.com/kb/817379/en-us

Hope this helps. If you have any question, please feel free to let me know.

    Hi Lawma & Rock,

Base on our review Rock & Chaub infomation & links, i think Chaub information is the solutions. The Rock show the KB link seems to ask if your back-end Exchange server enabled FBA on OWA. It will not work the Activesync & OMA. it show how to fix this error & show OMA activesync does not work when FBA enabled on Back-end servers.
